AZ Medical Marijuana Card Requirements
There are two types of Qualified Patients: Adult Patients and Patients Under the Age of 18. For patients under 18, the patient’s custodial parent or legal guardian must be designated as his or her caregiver. Adult patients do not need to designate a caregiver. There are different requirements for an adult patient and patients under the age of 18.
The Arizona Medical Marijuana Act states that anyone who adheres to the following requirements cannot be penalized for the medical use of marijuana. The Act prohibits certain discriminatory practices if you are:
- 18 years or older (or have your legal guardian registered as your caregiver)
- Must have a valid Arizona Driver’s License or Arizona Identification Card
- Must have an Arizona residential address
- You must meet the strict requirements for having a qualifying condition (full list)
Arizona Medical Marijuana Availability
Marijuana may be obtained from a licensed and state authorized non-profit Arizona Medical Marijuana dispensary.
Medical Marijuana Card Documentation and Fees
Proper documentation of a qualifying condition is required. You can obtain and must provide your current medical records (past 12 months). Your physician can provide this free of charge, even if they are unwilling to write you a recommendation for Medical Marijuana.
If you are unable to meet this requirement, certain licensed medical marijuana clinics may be able to recommend another consulting physician.
The Arizona Department of Health Services will review your application and issue a medical marijuana card within 7 to 10 days. This card is valid for one year from date of issue. Arizona Department of Health Services will send you a 90 day notice before your card expires to allow you time to reapply.
In Arizona, the annual fee for a new or renewal medical marijuana card is $150. (Current as of April 2014) Some qualifying patients may be eligible to pay $75 for initial and renewal cards if they currently participate in the Supplemental Nutrition Assistance Program (SNAP)
